Applications are dealt with by the local council that you live in and you can make the application online or you can ask them to send you a form.
They may ask for a patient summary which we can provide free of charge.
Sometimes they may need more specific information and why it affects you.
This would involve you asking for a letter from your GP and there is a fee of £40 for doing this non NHS work for you.
Medicals for Taxi/HGV/Seafarers/D4 DVLA etc.
You will need to have a vision assessment at an optician before seeing the GP for a medical. When you are ready to make an appointment with your GP, please telephone the surgery and ask to speak to Alison Shaw who will arrange this for you outside of usual NHS surgery times. There is a fee of £150 for doing a non-NHS medical such as these and is payable on the day of the appointment.
Please check with your insurance company if they need a letter from your GP or if they will need a form comto be completed which is more common.
The form will need to be submitted to the surgery so your GP can complete it and there is a fee of £40 for doing this non-NHS for you.
Please allow up to 28 days for the GP to do this non-NHS work.

These can be needed for all sorts of reasons so please put as much detail in your request as possible please.

Request for medical notes
You can also apply for online access to your notes you can download the NHS App which will also give you access to some of your notes and results.
Patients may need medical notes for lots of reasons but the request does need to be in writing please.
You may only need a copy of specific problems so please make sure you make this clear.
Please allow up to 28 days for this non-NHS work to be done.
Forms that need completing by your GP
Please submit your forms to the surgery and they will be passed to your GP to complete. There is a fee of £40 for doing this and please allow up to 28 days for the GP to do this non-NHS work. Some forms mention examination and in this case the GP will need to see you but you will be contacted about this to make an appointment outside of usual NHS Surgery times. There is a fee of £60 if you need to be seen regarding a form like this ~ such as being fit for a sporting event.
These are quite specific forms and can be for a new application for a firearms licence or a renewal of your firearms licence.
Please submit your forms to the surgery with your part signed.
